Cancelation requested month ago

I have called and requested more than once last year to cancel my subscription with Octotel at my previous address the agents keeps telling me someone will call my back but I have yet to receive a call back this is now 4 month down the line an they are still billing me and having fees charged on my back account this is not acceptable can someone please cancel my suscription immidiatly .
