Upgrade incomplete and no service

I upgraded my account 3 weeks ago. I received a confirmation mail but I am still waiting for my account to replect the upgrade and for delivery of the wifi modem. Agent chat sessions "mysteriously" closes after confirmation of security details and stating my query. I have various transcripts of these. When I contact the call centre the agents response is that they will get the relevant department to contact me - which never happens.
