Unauthorized debit order and a **** contract

I am very much upset with Telkom, They have been sending my husband messages saying I am owing on my contract, I don't have a contract with them, I went to Telkom Wonderpark and I discovered that a contract was opened under my ID however the name is not mine. They advised that I do an affidavit and submit it with my ID copy. I did that, weeks later I went to check how far the matter was, I was told that the person who took my stuff didn't log a claim. So now I have to redo the whole thing again and at the same time there is lost of money being deducted from my account by Telkom that I didn't authorize. Telkom staff i'm not happy with them. Can they please resolve this.
