Telkom won't cancel my ADSL

I have been trying to cancel an application for ADSL since May.!! Have been going to the Century city branch where they give me a form and do nothing to action the cancelation.

Am getting a bill close to R2000 for a service that I don't have. It's ridiculous how many times I have been. My account no is ********** 47.

This is appalling service Telkom??
