Telkom won't cancel my ADSL despite my many requests

In late April 2016 I sent through the official cancellation form to Telkom to cancel my landline and ADSL service. It was difficult to get hold of the correct form and after sending it to Telkom-provided email addresses and it bouncing back, I eventually found the correct one and sent it through. I was then told that it would take 25 days to be cancelled, so I was charged a debit order in May, even though I did not use the service that month. The call centre said that I could claim that debit order back when the cancellation had been processed and I also requested that the call centre person process the cancellation to ensure that it would go through. I was then charged a debit order in June, after the date I was told the service would be cancelled. I went to the bank and had them reverse the debit order and went into the Stadium-on-Main (Claremont) Telkom branch to confirm that the cancellation had been put through. The friendly staff member said that no cancellation order had been listed on my account and he proceeded to put through a new cancellation order. I was then sent a Telkom statement in July, where they added back the debit order amount the bank reversed for me in June and Telkom added the debit order for July too. So I sent the cancellation form again. Every time the cancellation form is sent, an automated email is returned and no result has been achieved thus far. Telkom proceeded to send me a debit order statement in August too! I have tried every possible official way to cancel this account yet it seems impossible. Even though I changed my bank account details so they haven't been able to deduced money, I don't want this looking bad on my credit record. I have not used the service since April and I have emailed Telkom via email addresses and their form on their website and yet no result. What on earth does one have to do to get this service cancelled?
