Telkom ADSL and line rental not cancelled

We have been trying to close this telkom adsl account for the last 3 months. All the relevant forms have been completed and submitted to Telkom. We have personally also been to the telkom store to get this sorted. We have moved to a different city but yet the telkom line is still open and gets billed even though it has been cancelled.

I am not sure why the staff at Telkom are not doing their jobs.
