Please remove your company from my credit record

Hi - I have paid my account, signed a document for cancellation , i was told that everything is in order and my account has been closed. But when i do my credit report. i see it says I still pay telkom R795 every month. This decrease my credit score . So please can you urgently remove yourselves from my credit report. Please.
