Please cancel our broadband connection account

Good day I hope you will be able to assist with this matter.My husband would like to cancel his broadband account with Telkom we tried multiple times with no luck we emailed a cancellation form which we got online we tried the online process but didn't work we didn't receive any further information about this issue.
