Loss of contract application forms

I went to telkom on 29 December to apply for a new contract for which my company was going to pay the phone as an expense. I was made to complete business application forms and beacuse the initial phone I had requested was out of stock my application was witheld. Due to the delay I decided to change the fone to one which was in stock and was told that my application will be processed within 72 hours. This was in january 2016 and until now I still have not received any communication from telkom. I then went in on thursday the 18th of February and was told that my paperwork is lost and they could not find my application forms after having waited for 2 hours in the shop. The manager also refused to listen to my complaints and said he would call me back as he was supposedly on a conference call but to this day the manager still has not addressed my complaint. Someone must take responsibility and find the forms to process them.
