Line cancellation hell, 8 months, many branch visits, please help!

We have been trying to cancel our landline since August 2017. Despite multiple branch visits, and multiple escalations, our line is still being billed. We have been paying all the way to November, so there is no way we still owe money.

Process this immediately or my next stop is the Ombudsman.
