Insurance I never signed up for

Bought a phone a month ago and did a quote for insurance and never took it coz it was too expensive. Now my statement comes with insurance. I have called million times and still nobody knows how to do it. I’m definitely take this case to the ombudsman because I don’t even know why that stupid agent sign me up
