How to stop debit order for service they cancelled

Hi. For the last couple years we tried to stop a debit order for internet services from telkom. Our adsl line was cancelled by telkom because they don't want to renew ****** cables anymore. That was about seven years ago. They said they will cancel my nr and stop account. I haven't check my bank statements because bookkeeper does the work. When he died and we had to start with new accountant we pick up that telkom still take R59 every month. Since then we regularly stop the payment at the bank but still no stopping off debit order.
Telkom closed its offices in Tzaneen so we can't go there again to try and stop this debit order.

Now I want to find out how we can stop this blatant theft. And how do I get all my money back. I can't stop it from my side because the bank just say they can reverse it but every month we have to go to the bank to do it.
Copied usage report of 2011
A friendly notification that according to our records your online usage as at [2011-06-18 23:59:59] reflected the following: Username: [Email Removed]
A. Data Allocated (Default Package) - 1.00GB
B. Additional Requested Out Of Bundle GB - 0Bytes or [0.00GB]
C. Total TopUp Purchased - 0.00GB
