How is TELKOM's accounts admin this bad?

We closed our accounts 13 YEARS ago, after settling the account 3 TIMES due to their poor administration. Last month on reviewing my credit score I discovered that I have an outstanding monthly amount of over R2000 not being paid? I have no idea of how this is possible after not having an account with them for at least 12-13 years?
