How do I get rid of HELLKOM?

My fixed line was taken out beginning of DEC2015 by lightning. This meant no DSL, which was the primary use. Despite logging a call they did not respond until JAN2016. By this time I have requested that all fixed line services to my address be cancelled and the credit on my account paid back. I have send the relevant documentation to [Email Removed] 4 times and still the account is active on my name. How deep does the incompetence run in this company? HOW DO I GET RID OF YOU AND GET MY MONEY BACK?
