***** on my Cheque Account for a contract I do not own

Please find attached affidavit for ***** case with Telkom as per the telephonic conversation with Portia (ref: ********** 62) on the 28-06-2018.

I am being debited different amounts since last year for a phone contract under the name of Uvendrie Coopsamy which I have no relation to.

The matter is ********** as I do not reflect on the Telkom systems for a phone contract or any other product and request a refund for all the debit orders which was taken from my cheque account.

Your urgent assistance would be appreciated.
