Contract not cancelled after 6 (SIX) months

We had a LTE contract for years and in January we have changed to fibre. To this date, in spite of numerous e-mails sent to ********** with the completed forms, calls to the callcentre, visits to the shop, the contract was not cancelled. PLEASE help us cancel our contract Thanks, MM
