Cant cancel a deceased mans telephone account

I have been trying to cancel my late fathers telephone line since July, i have sent death certificate numerous times and have multiple reference numbers but 5 months later i am still nowhere. i have even emailed individuals within the organisation who have promised to assist but nothing.
