Cancellation Request ignored and now calling attorneys

Asked CS to cancel telephone line, told to go online. They even printed instructions for me, which i followed. Now 2 month on they still have not cancelled my phone lines and now treating me with attorneys for debt that should have never been if they just did their job. And this not issolated case my dad's phone line was also requested to be cancelled and that has not been done either
