cancellation of contract not done tried twice

<p>I had a contract with you it ended 2014 however I was informed I had to complete a cancellation form which I did twice via email and then I went in personally I just found out now nothing has been done I am really not happy with your services and lush I opened a new contract now I'm being billed for two items</p>
