Breach of Contract via Telephone Sale

I am alarmed to receive my account with additional amounts debited without any disclosure via the telephonic sale and my contractual agreement. This is a breach of contract. Also main contract number of Telkom does not even have the supplier Mondi's number recorded. My accounts states contract was face to face. I was billed for August an amount of R414.25 yet my debit order contract is for R129.00 per telephonic sale, why? Telkom indicating debit order did not go through which is ridiculous as I have funds available always. Did not agree to itemized billing paper or CLI presentation. I wish to cancel this contract with immediate effect due misrepresentation of the agreement being a total of R129.00 per month.
