Account issues

I have a contract phone with Telkom as I had taken one of their packages in February this year, in June 2 cellphone lines and an insurance were taken with my name and added on to my account through their call centre without my concern and approval. I then went to one of the stores to report the matter and was advised to do and affidavit and also bring a copy of my ID and then they will log a case with their ***** department, the 2 lines were suspended and case was logged but since then I have not heard from Telkom and getting billed for the devices or packages that I am not using.
