Refund & Cancelation issues

Having an issue with canceling my month to month contract (R699). I tried to cancel on the 23rd of September and had my device collected on the 9th of October. And now on the 21st of October I was debited again. I would like a refund immediately as the discussions I have had thus far where assuring but there has been no follow through. Very poor service. Total amount to be refunded is R699 x 2 = R1398. Please stop all debit orders immediately.
