forms

I am currently on maternity leave and have on numerous occasions requested that the company complete a UI-19 form for me as I left the company in 2013.I firstly called Quest in Cape Town and was told to call Joburg as they would be able to assist me with the forms.I have been requesting this informations from february 2016 as the Department of Labour would not accept my Uif maternity forms without the companies UI-19 form. It's almost time for Me to return back to work in May 2016 and still have not been able to claim my maternity benefits.I am disgusted that up to now I have not had any feedback from Quest and when I spoke to them back in February 2016 I was told the form would be sent in 7 days time.I feel this company do not care about there previous employees, they should've completed this document when I left the company.I have also contacted another previous employer Energy at Work also based in Joburg to complete my UI-19 form and have still not received any feedback.I feel that because these companies are delaying sending the forms I'll probably end up returning back to work without receiving any maternity pay out.and I'm sure I'm not the only one suffering.Not happy at al
