Afrihost.com - locked my fibre line and were never my service provider

Vumatel installed fibre to my house. I called Afrihost to be my service provider one Saturday early last year and they said they could install the next week, I inadvertently gave them my banking details. When I called on the Monday they said there would be a wait of 3 weeks so I cancelled. In the meantime without me filling in any forms they debited my bank account. I had the bank reverse the debit and prevent any further debits going through. They have now locked my fibre and am unable to use another service provider. The information requested is beyond belief and is very private for someone who has never used their service.
