Zone Fitness Account created without my consent

Please note that I have never had a Zone Fitness account. I did a enquiry last year for one, however I never signed up for the account.

My instruction to the sales person was that i would apply for a Vitality membership from my Discovery Medical Aid in order to get the discounted offer through them.

I have to date not app**** for the Vitality Group membership or benefits hence me not applying for the Zone Fitness membership. Now I am getting phone calls about an account being handed over.

Please show me where I signed up for this account. I have not received an email from zone fitness for any account as well. Neither have I ever set foot in any zone fitness establishment.
