Wrongly listed on credit bureau by Vodacom

I’ve learned yesterday that Vodacom has listed me on credit bureau for a cellphone contract that I have never taken. I’ve never had a cellphone contract with Vodacom before how come am I listed on credit bureau for something I’ve never done? This says so much about their vetting process and security. How did they issue the Vodacom contract? Where did I sign? This is extremely frustrating
