Vodacom insurance and repairs are useless

My phone's screen broke, thankfully I have a phone insurance policy with Vodacom.

So i goto the vodacom store and fill the required paperwork, including the claim form in the beginning of December
Vodacom phones me just before christmas to tell me that i must pay R6000 to have my phone fixed or they will be sending it back. I told the phone operator that i had submitted a claim form for insurance to cover it. He tells me he has no record of it.
The phone was then sent back unrepaired a few days later

So i filled out all the paperwork again and resubmitted it for repairs, making sure the claim form is stapled to the other forms.
After 3 weeks of waiting I phoned Vodacom to find out whats happening.
They tell me again that there was no claim form attached with the repair job. and that they are sending it back unrepaired if i don't accept the R6000 quote for repairs.

Seriously. its been almost 3 months with no phone and and they can't even coordinate between repairs and insurance properly.

What the hell kind of service is this.
Please fix this asap
