Upgrade not processed and no accountability from staff

I processed a SIM-only upgrade on 20 Sep 2016 via phone to be effective 1 Oct 2016.

Despite completing the process telephonically and receiving confirmation from the consultant that the upgrade was completed, nothing changed on 1 Oct. When I called to enquire I was told the "quote" expired, hence the upgrade didn't go through.

Doing the upgrade again with effect 1 Nov would be fine, but because the Sim-only deal doubles my minutes, data and SMS, I'm now liable for usage that would have fallen within those thresholds. When I requested that the upgrade be backdated and any errant billing be corrected I was told it could not be done, and that it was I that was at fault, because I should have known that a quote needs to be signed (never mind I never received it).

Those involved:

Macallister (spelling?) - consultant
Team leader - can't recall name

Lennon - Call centre manager

The latest individual I spoke to, Lennon, promised me a week ago to have it rectified and to keep me up to date. I have heard nothing from him. Added to which, his tone was flippant and arrogant, and suggestive that I was being a nuisance for wasting his time.

I certainly don't expect amazing service from Vodacom - after all I'm only with them because of the network - but this is beyond the pale.

I was contacted by someone from the HP team and I was assured that the upgrade would now be active from 1 Nov and and the usage for October would be reviewed to determine credit.
I just logged into my Vodacom profile and it still shows that my contract is due for upgrade?

Why must I now be subjected to R150 upgrade fee when I processed the upgrade via the call centre?
And what about usage for the month of November, as the upgrade will now only be effective from 1 Dec?
