How to contact the ombudsman?

I had a contract with vodacom, I gave you my banking details and you read it back to me as confirmation, then few months later my services were suspended because you had the incorrect banking details. The person who called read my banking details and I said that's incorrect.. Anyways they then ''fixed'' the error on that call. A month later I get another call to say that you couldn't debit. Again the banking details were wrong... I then got an email from Vodacom asking me to make payment arrangements, I then gave my banking details YET again and arranged for debit orders to happen. At this stage, my data was already suspended. I still have emails as proof and you still have call recordings as proof. No debit ever went off and this arrangement was made in March but you have the nerve to hand me over to debt collectors. ... Please forward me all the notes, call recording, etc as I need it to open a valid case with the Telecommunication ombudsman. Thank
