Got insurance but losing second phone

Good day,

I sent in my Samsung S4 for repairs a few years ago and never received feedback, I got a new phone, (Samsung A5) then and moved my insurance to it in store. My A5 got damaged and I sent it in for repairs only to here it is not insured. My question therefor is, what device is in fact insured, and if not the A5, why didn't they switch it. Also, the insurance papers I received now is not even on my name. What is my steps to take, should I lay a charge of theft forthe S4 and then a complaint about my A5 because you never activated my insurance on the device even after I was told it is fine? And if no action is received on the A5 take further legal action?

In fact, what I expect is, you give me back my S4 (repaired as it was insured), and if not possible, replace it (if you cannot give me another S4, I want a market relevant phone for the price it was worth at that time), secondly, repair my A5 (as it is insured) and send it back to me.

Regards
