Contract taken out on my name

According to my invoices a 24 month R598 \3G laptop installment"contract was taken out on my name in November. I had a 3G contract on cell number *** that I didn't want renewed and it would have expired in November 2014. That I wanted to cancel this existing contract in November. I was told by Vodacom customer care and their retentions and cancellations department that I had to get an affidavit from the police before they will do anything. This was the 14th of November. So far nobody from Vodacom has contacted me and so far two amounts of R598 has been debited from my bank account.
