Cellphone contract taken without my consent

Hi my name is Simangele Khoza. I had a contract with vodacom that was suppose to end in January this year. Last year october/November a S5 was taken under under my name without my consent. Now vodacom claims I owe them 7000 for the phone. I went during December to the vodacom outlet at tsakane mall to enquire about the matter. The sales consultant confirmed that an S5 on contract had been taken under my name. The sales consultant than adviced me to write an affidavit stating that ***** had been committed and I should fax the affidavit to vodacom. I faxed the affidavit in January and I was told the matter would be resolved in 24 hours. As soon as I faxed the affidavit, I phoned vodacom to confirm if vodacom had received the affidavit. The call centre agent confirmed they had received it and they would send an sms with 24 hours. Since they day I haven't received any sms and when I call the customer care line Im told they are still investigating and I should be patient. it's the been 3 months since I faxed the affidavit and still no response from vodacom.
