Cell phone insurance that is unauthorised

I was a victim of identity *****, a cellphone contract was taken out using my details, Vodacom didn't bother to verify my details as I am listed under the South African ***** Prevention Service. The contract was eventually cancelled but now I am dealing with unauthorized debit orders for a Voda sure cellphone insurance. I spoke to a Kamogelo on the 27th of March and he assured me he cancelled the insurance. A month later another debit order went of from my account for the same insurance policy. I am tired of unauthorised debit orders and having my name tarnished for things I didn't apply for.
