Cancellation of debit order

I am struggeling now for months to cancell my debit order at Vodacom. First I was said that I need to do an avidavit in order to cancell my debit order from my bank. Then I was told to complete cancellation forms and submit it to Vodacom. I've submitted the forms on 5 April 2017 at 15:05 via fax as requested. On 12 April 2017 I followed up and it was said they did not receive any forms. I've e-mailed the form again to Muneeb at Vodacom on 12 April 2017 at 10:47. On the same date at 14:00 I receive a confirmation sms, reference 1- ********** ********** . End of April 2017 my debit order went off again. I've requested them to cancell my debit order as I am receiving salary on different dates and will be easier to make EFT payment, as I did now for the last 3 or 4 months. Everytime I am charged bankfees if the debit order gets rejected. I do not now why this are a issue. I need Vodacom to cancell my debit order immediately.
