Blacklisting of a ****** phone

Can someone please assist me to Blacklist a ****** phone, I have called the Customer Service was referred to a branch but I could not get assistance. I called cancellation trying to cancel the contract and I was referred back to Customer Service, I have now completed the blacklisting form but I have not heard any response and I have been trying the Customer Centre number and there is still no progress.
