4 times I have requested a call and Vodacom is not willing to give it to me

Please can I have the forms to complete to get a copy of the last call that they made with me to upgrade my phone. I want this call, if you do not supply me with the call I am going to report this to the information regulator. The reason I am looking for the call is that I was told I can upgrade in July this year, now I am being told next year April or October. I have asked for this call 4 times now.
