Still waiting for someone to help me black list my phone - i got no forms

Alicia, Please help me, i never got your form for blacklisting the previous phone i had, I have no details about my previous phone. it was the Nokia Lumia that i got from you guys in 2015. Please i need this phone blacklisted and i never got the forms. Please its very urgent. please email me the forms i need this phone blacklisted. Please! Not my 2017 contract phone, the one before that. Please email me on ********** or just help me as this serves as proof that i want my phone from 2015 blacklisted
