An Uber driver drove out of a driveway, over 2 lanes of traffic and crushed into my car

An Uber driver drove out of a driveway, over 2 lanes of traffic and crushed into my car. He afterwards tried to intimidate me. I had to call Metro. I tried to log a complaint on the Uber App and online. Because I wasn't in the Uber and had no "Ride", I can't log a complaint or report the accident. Online, I can fill out all the forms, but after confirming numerous times that "I am not a robot" and clicking numerous "cars", "bikes" and "crosswalks", I can't Submit the claim.
