How Does a Bank Loose Financial Records

I've been emailing & calling TymeBank this whole week requesting my Tax certificate for 2*** instead they keep on sending a Tax certificate for 2***. They completely ignored the last email I sent breaking their own SLA

As a result, I can not submit my tax returns. I'm upset because how does a bank loose financial records
