Wrong debit order dates

Ive had this account for almost a year and ibe had problems with my debit orders not going through. Time and time again went im store to set up mew debit orders. Today ive received a notification from the bank that they made a debit which is the incorrect date i receive my salary firstly and secondly how do they make changes to my account without my consent?
