***** on account

I app**** for an account online and was advised that my card would be delivered. To date I've not received the card, but received a survey for a recent purchase at Daniel Hecter Balfour Park. When I called customer service to query this, they indicated that purchase was done in store and that they would send me a declaration form to dispute those purchases because I've only been purchasing online. I'm still waiting for the form.
