The pre-authorisation request was reviewed by the Medical Advisor and confirmed the benefits provided on your plan only cover Prescribed Minimum Benefits (PMB) with-in State hospitals/facilities as the Designated Service Provider (DSP).
Please consult with a specialist at a State hospital/facility, as discussed with the case manager. If the treatment and condition is PMB-related, Transmed will fund the operation.
