Wrong info and enquires that i dont know

Ive Got two enquiries with standard bank, which ive never app**** a loan with them, upon me calling them they trafered me to their debt review department which claim that am under deb review of which am not i was once under and i managed to complete the process all credit providers updated my profile including NCr, however i do not understand why with them it still showing debt review flag and why did they put two enquires under my name without applying credit with them.
