Unauthorized withdrawals and charges

So on Monday I was at standard bank century city to get a few things done regarding my 32-day savings account. I was being assisted by a guy name Deano. After more than an hour (why it took so long I don't know), I eventually walked out the bank and received two text messages stating that my contact details have been amended, in which I did not request for any contact details be amended by any means.

Yesterday morning I decided to log onto my banking app and noticed that my balance is lower than it should be. I decide to check my transactions history and noticed that R130 was used to issue a replacement bank card (which I did not request) and R1000 was withdrawn at an atm in century city (which I did not do or authorise). I never received ANY notification on these charges. I decided to get in contact with standard and the same day i get a call from the manager at this branch stating that everything was a mistake made by this consultant named Deano. After digging a bit deeper trying to figure out how a mistake like this can possibly be made by a consultant, I asked if this employee still works there in which she answered yes. A R1000 withdrawal fee amount has been reversed and so was the R130 for the replacement card that was issued. This morning however I get charged R130 AGAIN. Note, through all of this I did not receive a single notification via sms as I normally would, which tells me contact details have indeed been tampered with, and my question is WHY???

All of this sounds pretty much like ***** to me. The question Is, HOW DO I PROCEED?? Any advice would be appreciated. Thanks

This morning I visited the branch again and requested to speak to the branch manager. I was told the branch manager is not available and that I could speak to the floor manager. (All of the below has been recorded on my phone). According to the floor manager, the consultant, Deano, has made an error by issuing a replacement card in error (In which I've been charged R130 for) and somehow having a R1000 withdrawal made from my bank account, without my authorization. My contact details have also been amended in 'error', hence why I did not receive any notifications with regards to these transactions. How does a tenured consultant, someone that's been working for the bank for 5 years, make an 'error' like this? This is completely unacceptable and makes it really difficult to trust a bank I've been banking with for years. THIS IS *****!! And it needs to be dealt with accordingly.
