Unapproved De*****ek Mandate

I just received an sms from bank that a new debit order mandate was added on my account without me approving it, I want to know how is it possible that such mandate be approved by the bank without my knowledge or approval?

when I called this MaxLaw company that said they had cancelled it yet I still have not received any sms from the bank that the mandate has been cancelled.
