Standard Ignore-ance

I bought a motorcycle from a dealership and I financed it through SB-VAF a month ago. I wanted the Top-up insurance and Credit Life through SB-VAF not the dealership. But Simphiwe Mkhonza keeps sending me blank insurance forms that I don't know how to fill in some of the details thereof and on top of that the scanned quality thereof is illegible! So long story short I've had the motorcycle for a month, but still have not been able to arrange the insurance I mentioned through Standard Bank VAF and SImphiwe just sends me blank forms to fill in. Eventually I managed to get the Top-Up forms submitted, but can't get the Credit Life signed-up due to Simphiwe's ignorance.<br>
