Standard Bank Credit Card Division committing *****

I had a credit card back in 2010 with them. I went into debt consolidation and my credit card was in the monthly debt consolidation payment. It was fully paid off 2016. So why my credit report reflects that my standard bank credit card is in use. When u enter debt consolidation automatically you can't use that card. Need answer.
