Lost my Fica document 2 times in one year

Last year I wanted to increase my STD bank EFT limit but was told my fica was unclear although they have my FICA from the house bond I have with them.<br> <br> I had to go into the bank and give them my ID and W/E account again.<br> <br> Now this year same issue, want to increase my limit on EFT tell me I need to take FICA documents the one they have is unclear. <br> <br> I am tired of this now, I want STD bank to either come and fetch my Rates and copy of my ID from my OR get someone to call me to close my homelone and accounts with them.
