Lack of response and unauthorised card upgrade

For over 2 months I've been dealing with the bank to address what I believe is an unauthorised upgrade to my credit card.

This started in November 2014 when I received a phone call saying I'll be upgraded to a high card program, with the same fees. I was suspicious of the call as (1)the phone number was associated with a marketing company, and not directly with Standard Bank, (2) the way questions were asked came across as someone trying to obtain sensitive information from me. I did not divulge what I believed was sensitive information.

The next day I emailed the card division to enquire about the authenticity of the call. I did not get an answer to my question, even after 2 follow up emails.

In December 2014 I then started receiving statements which indicated I have a new card program and the fee was higher. After several more calls to the bank and indications from the call centre (on 2 separate occasions) that the a representative from the bank would call me back, nothing has happened.

No one called me back as promised, and all I have now is 6 different reference numbers, a credit card that does not work and new statements arriving every month. Please help me change this.
