Debt caused by bank

I have been a Standard Bank client for 10 years - In July 2016 I app**** for a credit card for our honeymoon expenses, and to my surprise I was declined. With this I forgot about it and moved on. Today (17-01-2017) I went into a branch to apply for a personal loan, where a very helpful consultant informed me that I am not approved as I owe the bank R500 for credit card service fees that have accumulated since July 2016, and that I have been listed on ITC. She phoned the call centre I assume, and requested for all credit card fees to be reversed and the account closed, as I was not aware that I even had a credit card, or was informed neither by email, telephonically or via post that I have credit card fees outstanding. This has now affected my credit score, which is worrying. I have now re-app**** and the consultant said that she will send the documents with a motivation to approve the personal loan which may take up to 72 hours, then I will only be contacted regarding my loan. This is not convenient as the personal loan is to get myself and my family through the month.
